Cat-scratch disease was initially considered because of the granulomatous lymphadenitis and cat exposure. When the serology was negative for the causative agent of cat-scratch disease, Bartonella henselae, an infectious disease consultation was sought. Chronic granulomatous disease (CGD) was suspected and confirmed by a nonreactive nitroblue tetrazolium (NBT) dye test. Genetic testing of the child showed an X-linked gp91~(phox) deficiency that is associated with approximately 70% of CGD cases; the remaining 30% of CGD cases are autosomal recessive. Of the females who give birth to males with X-linked CGD, approximately 10% have discoid lupus erythematosus [1].
