INTRODUCTION Lower leg pain in young athletes is generally a relatively benign condition with a broad differential diagnosis of calf muscle, tendon, ligament, joint, or even bone injury resulting from exercise. Vascular disease is rare in these young individuals. However, our case illustrates that the differential diagnosis should include popliteal artery entrapment syndrome in cases of calf claudication (lower leg pain) associated with coldness, blanching, and numbness of the foot after physical activities. We present a unique case of a young fitness athlete with initially supposed harmless lower leg pain and discomfort. Unfortunately, it turned into serious morbidity with amputation of 4 toes as a result of a progressive course of popliteal artery entrapment syndrome.
