Insulation abrasion is the most common cause of implantable cardioverter defibrillator (ICD) lead failure. Riata leads are prone to a unique form of failure mechanism with abrasions of the silicone insulation. In view of the national Netherlands Heart Rhythm Association recall guidelines for Riata leads, we performed additional investigations in all patients who received Riata leads in our hospital. A 75-year-old male patient underwent cardiac resynchronization therapy defibrillator (CRT-D) implantation in 2007; the right ventricular ICD lead was a single-coil 7-Fr St. Jude Riata model 7002.
