Clustering of Ventricular Arrhythmias: A Therapeutic Opportunity or Strictly a Marker of Poor Prognosis?

In this issue of the Journal, Lunati and colleagues report on clustering of ventricular arrhythmias in patients with biven-tricular implantable cardioverter denbrnlators (ICDs) who were implanted as part of the InSync Italian ICD Registry.1 While clustering of arrhythmia episodes has been studied by other investigators,2'3 the unique feature of the present study is its exploration of the occurrence of clustering in heart failure patients with both ischemic and nonischemic cardiomy-opathy, as well as in patients with both primary and secondary indications for ICDs with resynchronization therapy.
