Purpura in a patient receiving vancomycin: a leukoclastic vasculitis?

Infective endocarditis (IE) is associated with considerable mortality. The continuing evolution of antimicrobial resistance among common pathogens that cause IE creates additional therapeutic issues. Staphylococcus aureus is now the most common cause of IE. As the use of vancomycin increases, it is likely that the incidence of rare side effects will increase too. The authors report a patient with a rare vancomycin-induced side effect. A 38-year-old woman was admitted to the hospital with fever. Medical history included atrial fibrillation and a recent cerebrovascular accident. One month earlier she underwent cardiothoracic surgery, including an aortic valve and mitral valve replacement.
