Pro: continuation of aspirin/clopidogrel for cardiac surgery.

THERE IS AN URGENT NEED for well-founded guidelines for the perioperative management of patients on clopidogrel. The key question here concerns patients on dual antiplatelet treatment who are in urgent need of surgery. Should clopidogrel be discontinued for >5 days to decrease perioperative bleeding in those patients and by doing so increase the risk of a major preoperative cardiovascular (CV) event? To answer this question, clinicians have to clarify the reasons why the drugs are taken and scrutinize the data that can guide their strategy.
