Introduction or a pharmaceutical relay between the hospital and city pharmacies: application to the patients treated by oral anticoagulants

Oral anticoagulants are directly involved in many iatrogenic accidents. To reduce them, the therapeutic education during hospitalisation "at the patient's bed" can be useful. In order to ensure the good patient understanding, the pharmaceutical follow-up must be dispensed from the exit of the hospital by the private pharmacy. Our study consisted in evaluating the impact of the education and the patients' understanding of the transmitted informations about oral anticoagulants during their hospitalization in a cardiology unit. Each patient's pharmacist was contacted to explain our study and obtain his agreement. An education report and a standard evaluation questionnaire were mailed and had to be remailed to the hospital pharmacy after treatment dispensation. Thus an information feedback was possible especially for the items memorized by the patient and pharmacists comments. Fifty-six patients were included during six months. No participation refusal was emitted by any pharmacists. Twenty-three pharmacies answered spontaneously and 11 were phoned back. The originality of this study was the attempt to create a relay between hospital and city pharmacies in order to ensure the continuity of the pharmaceutical follow-up. This study is now extended to several clinical units where pharmaceutical presence will be reinforced.
