Rehabilitation of aged persons after stroke-Combination of neurological expertise and geriatric competency

As a consequence of the increasing number of elderly stroke patients there will be a high need of rehabilitation in the tfuture. For the treatment of elderly stroke patients considering both, neurological and geriatric expertise, is necessary. Even in advanced age successful rehabilitation is possible, if impairments are considered in multidimensional diagnostic and therapeutic process, and if existing ressources are identified for the support of the rehabilitation process. Realistic goals and the availability of an adequate rehabilitation setting are important. Particularly in elderly stroke patients an individual discharge management, integrating further outpatient interventions, is relevant for the sustainability of rehabilitation success.
