While standing on a drill platform of a continuous miner an operator was hit by a slab of rib coal that pushed him against the side of the machine. Underground power tripped when an 11kV cable was damaged. A fault on the underground 11kV switchboard enabled the circuit breaker to remain closed. The trip was reset at the surface three times causing arcing and fire at the cable damage site. An employee received an electric shock while inserting the plug into a 240V outlet in an area where he was using a high pressure water cleaner.
