ISMP Adverse Drug Reactions

A very important retrospective literature review describing the underreporting of harm in reports of randomized controlled trials (RCTs) was recently published in the Archives of Internal Medicine. An excellent editorial accompanies this review and asks the question, "Why is it, then, that reporting of adverse events is often insufficient or misleading?"
