When Frank Sykes was an HSE principal inspector, one of his office-based colleagues asked to be taken out to visit "somewhere awful". (This was during the days when inspectors were less restricted in visiting businesses.) Sykes picked an engineering company that employed around 25 people. It was notorious and had been served with a number of notices in the preceding years. But when Sykes and his colleague arrived at the factory, it was not as they expected.
