A FATAL DECISION appears to have been first produced in 1997 for the Thames Water Health and Safety Group, presumably as a training sup-port for the Confined Spaces Regulations which came out that year. That the video is now available to a wider audience is both timely and relevant. It is timely because the HSE is again raising awareness of the hazardous nature of confined space working given the 4 reported deaths in 2003. It is relevant because the ad-vice contained in the video would have prevented those and many other fatalities from occurring. There really is no excuse or justification for any-one to work in a confined space situation and be at risk of death. The fact that fatalities continue to occur points out the very poor understanding of the risks and requisite safety systems that should be implemented.
