First of all, we appreciate Dr. Esquinas and Dr. Dikmen for their great interest in our paper [1]. They wonder that the relationship between unplanned extubation (UE) and in-hospital mortality is an accidental association or cause in critical surgical patients. We do not think that UE per se is a direct cause of hospital mortality.
