Tesoro fined $2.39MM for deadly, 'preventable' refinery blast.

Following a six-month investigation, Washington state's Dept of Labor & Industries (L&I) determined that die April 2 explosion and fire that killed seven workers at the US refiner's 113.3K-b/d Anacortes, Washington (US) refinery was preventable. According to regulators, the deadly incident could have been avoided if equipment was properly tested and if TSO followed other safety rules. L&T cited Tesoro for 39 "willful" violations and five "serious" infractions of workplace safety and health regulations, for a total fine of nearly $2.4MM. The accident is the worst industrial disaster in the 37 years diat L&I has been enforcing the state's workplace safety law. TSO has 15 working days from the Oct. 4 announcement to appeal the findings, if it chooses to do so.
