All too often after a wellsite incident (not accident) has occurred, we hear the contractor's crew say, "The company man said if we didn't do it, he'd kick us off location" or, "We didn't know what to do given the situation." The most likely reasons for a crew to make these statements is usually two-fold: (1) The contractor company has not developed and established any standard operating procedures (SOPs) for performing the job at hand; and (2) if the SOP does exist, it has not been properly communicated to employees, or sufficient training has not taken place.
