The rare insertion of b4 with trifurcated portal vein in live donor.

Correct understanding of the anatomic variations of the vascular and biliary systems is important to avoid postoperative complications in live-donor hepatectomy. We reported biliary variations associated with trifurcated portal vein in right-lobe grafts (1,2). In general, anatomic variations requiring multiple vascular and biliary anastomoses are more frequently encountered in right-lobe graft than in left-lobe graft. We report here an extremely rare case of biliary anomaly in a left-lobe graft donor.
