Acute appendicitis in the immediate perioperative period following renal transplant.

To the Editor Acute abdominal pain in the immediate postoperative period following renal transplantation poses a diagnostic dilemma and is usually related to the transplant surgery itself. We report an adult patient who developed acute appendicitis within 7 days after a renal transplant, in the setting of a postoperative retroperitoneal bleed. The ultimate diagnosis was only made because of a high index of suspicion, given the many possible etiologies of the peritoneal symptoms. To our knowledge, this is the earliest case of post-transplant acute appendicitis reported to date.
