An evaluation of a clinic-based pharmacist and a medicines home delivery service from a transplant outpatient department

The Oxford Transplant Centre has over 900 patients with functioning grafts and performed nearly 100 renal transplants last year.It serves a population of around three million from six counties in south central England.After patients have been discharged,they continue to visit the centre's outpatient clinic for consultations and blood tests,and to obtain further supplies of medicines.
