Process Safety lessons from the BP Texas Refinery Incident and the Bhopal Disaster

The investigation report of the BP Texas Refinery incident that occurred in March 2005 has been issued by the US Chemical Safety and Hazard investigation Board(CSB).The incident killed 15 workers and injured 180 others.A reading of the report(www.csb.gov)indicates that many of the root causes of the incident are similar to the Bhopal Disaster that occurred 21 years before the BP incident.
