Formal clearance of axillary nodes in patients with suspected malignant infiltration has been a basic tenet of breast-cancer management for the past century. Non-resection of operable nodal disease with contemporaneous metastases and reliance on non-surgical modalities for eradication represents a fundamental paradigm shift in therapeutic approach.
