Folded Free Vascularized Fibular Grafts for the Reconstruction of Combined Segmental Bone Defects of Distal Tibia and Fibula

Combined distal tibial and fibular bone defects with a short distal fibular remnant and distal syndesmotic injury will result in ankle instability.1'2 When associated with long bone defect and extensive soft-tissue injury, it presents a significant challenge to the reconstructive surgeon. In this article, we present a case of composite bone and soft-tissue defects of the left lower leg involving both tibia (6 cm) and distal fibula (8 cm) successfully treated with a folded fibula osteoseptocutaneous flap from the right leg. Good bone union, ankle stability, and ankle motion were noted during the postoperative long-term follow-up.
