Although multifactorial in nature, the contribution of parenteral lipid formulations to the development of PN-related liver disease is now well established.4 The mechanism of injury is due, at least in part, to original soybean-based formulations favoring formation of proinflammatory signaling pathways. This resulted from formulations with high omega-6 fatty acid content, unfavorable ratios to omega-3, the presence of phytosterols, and low alpha-tocopherol content. Research to diminish this injury has focused primarily on altering the parenteral components themselves to diminish injury.
