We feel that the ideal method to substantiate any breast-milk mediated DENV spread to the neonate would be a biopsy from the breast because it would be possible to define the precise location of DENV RNA by an in situ analysis on paraffin-embedded tissues. During such an analysis the tissue architecture (lactiferous ducts, cellular lobules, loose inter-lobular stroma, myoepithelium, blood vessels and lymphatic tissue) would remain intact and thus enable the localization of DENV in individual breast component. This method was used on an 11-year-old Thai boy who died of dengue hemorrhagic fever. The tissues were treated by transcribing the viral RNA to DNA, followed by amplification using PCR and subsequent in situ hybridization. DENV RNA was detected in hepatocytes in the mid-zonal region of the liver, while scattered macrophages were also observed in the skin and lymph nodes[3].
