It was with great enthusiasm that I read "Blood Loss After Total Hip Replacement: A Prospective Randomized Study Between Wound Compression and Drainage" by Johansson el al (J Arthroplasty 20(8); 967). After reading the article, 1 realized that there was no true control in their study. The authors compared a drained wound with an undrained wound with the addition of compression. They did not include an undrained wound without compression, which would have been a true control over the variable of compression. Although intriguing, there can be no conclusion regarding compression without an undrained control group.The conclusion, that "wound compression may have the potential to replace the routine use of postoperative wound drainage after THA," is also naive in the fact that there have been several articles documenting no difference with or without using drains in total hip and knee surgery.
