Surgical treatment of subungual melanoma: A case treated with bone splitting of the distal phalanx

We thought that nonamputative resection of the distal phalanx (DP) would be insufficient and might not secure a sufficiently deep safety margin because of the clinical destruction of the nail. Therefore, under local anesthesia, the entire left thumbnail was excised en bloc with the nail unit and the dorsal side of the DP (Fig. 1b,c). The DP was resected with a bone-saw, and the resultant DP excisional defect was covered with an artificial dermis.
