Actinomycosis is a chronic, suppurative, granulomatous infection. Gram-positive, non-spore-forming anaerobic or microaerophilic rods, primarily from the genus Actinomycetes, are the responsible agents.1 Primary cutaneous actinomycosis of an extremity is a rare entity.2 We report two cases of primary cutaneous actinomycosis of the hand.The first case was a 12-year-old girl who presented with a 6-month history of chronic swelling at the dorsum of her right hand in the carpometacarpal area. She could not recall any trauma. At the physical examination, a nodular, mobile, non-tender mass surrounded by erythematous changes, 3 cm x 3 cm in size, was palpable on the right hand dorsum (Fig. 1a). No lymphadenopathy in the relevant lymph drainage areas was detected.
