A 59-year-old white man presented to his primary-care physician with the chief complaint of gradually increasing dyspnea and nonproductive cough. Symptoms were exaggerated during physical activity. He had no known pulmonary disease, although he was a former smoker with a 30-pack-year smoking history who quit 25 years previous. The patient's medical history was pertinent for hyperlipidemia for which he was well treated with statin therapy. He took no other medications and had no allergies.
