A 36-year-old woman with cough, positive purified protein derivative tuberculin skin test, and cavitary upper lobe consolidation.

A 36-year-old woman was referred to the pulmonary clinic for a persistent cough and a worsening abnormality on chest radiography over 4 years. The patient had emigrated from China approximately 20 years prior; during application for permanent residency in the United States, her purified protein derivative tuberculin skin test converted to positive, leading to chest imaging that revealed a left upper lobe opacity (Fig 1). The patient was completely asymptomatic at that time. Bronchoscopy done at that time without a biopsy was negative for acid-fast or other organisms.
