Internal herniation is a rare cause of intestinal obstruction. It may be spontaneous or acquired (after abdominal surgery or trauma). In internal herniation, herniation "into" the greater omentum resulting in intestinal strangulation has never been reported before. We present here a 40-year-old man who presented with intestinal obstruction and strangulation resulting from herniation of the small bowel "into" the greater omentum just below the left side of the transverse colon through a defect in its posterior leaf with a brief discussion of its pathogenesis.
