A bony left lung

A 48-year-old Chinese farmer was referred to our hospital due to progressive pain of the right leg for 6 months. ~(99m)Tc-methylene diphosphonate whole-body bone scan demonstrated an irregular area of intense uptake in the right distal femur, whereas no other abnormalities were shown (Figure 1A). The patient received right leg amputation, and histopathology showed osteosarcoma with adjacent tissue invasion. The patient went back to his countryside home and failed to follow our instruction to come to receive chemotherapy. Nearly 20 months later, a follow-up bone scan showed huge metastatic deposits in the left hemithorax (Figure 1B).
