Case 6: Maintenance Therapy for Simple Intermittent Gout

Patient AK is an obese 54-year-old white man, who works as a security guard. He was seen in the emergency room 12 days previously for the sudden onset of acute monoarthritis involving his right ankle. He had experienced no previous episodes. Synovial fluid aspiration was performed, and intracellular monosodium urate crystals were seen on polarized compensated microscopy. His laboratory tests in the emergency room showed a borderline high serum creatinine level of 1.4 mg/dL (124 pimol/L) and an elevated serum urate level of 7.4 mg/dL (0.44 mmol/L)
