Introduction: Margin status is considered one of the most important factors related to local recurrence (LR) following breast conserving therapy. What constitutes an appropriate margin remains controversial. The objectives of this systematic review were: (1) To evaluate the quality of the literature on margin status and LR; (2) To determine whether there is evidence supporting a minimum margin of excision greater than histologically negative margins. Methods: A literature search of MEDLINE, EMBASE and Web of Science, as well as bibliographic review of identified studies was conducted to find all papers specifically addressing the impact of margin status on LR of invasive breast cancer. Two reviewers evaluated the methodologic quality of the papers using a checklist based on previously published criteria.
