I congratulate Dr Jou and colleagues [1] for drawing attention to the important issue of Mate status. I completely agree that we should consider the importance of folic acid in pregnant Taiwanese, although it is a little late. Evaluation of the cost-effectiveness of a folic acid fortification policy and promotion of its supplementation are warranted in Taiwan. However, I think that the conclusion of the article that a public health policy or strategy "to increase the preconceptional use of folic acid is needed" in Taiwan is too strongly worded, because the paper did not give much evidence of this, even in the discussion section.
