Sir:The continuing quest to achieve optimal surgical facial rejuvenation in patients must address the individual patient's past and current medical history and financial limitations, together with the individual's degree of facial aging. Elderly obese patients are likely to present with associated health risks that may prohibit a prolonged operation. Limiting the surgical dissection to the neck will decrease the operative time and its associated risk factors, and thus reduce the risk of a potential complicated outcome. Since facial fat is abundant in this subset of patients, excess skin and moderate descent of underlying facial structures are camouflaged. The neck, however, requires skin excision after removal of underlying fat. Rejuvenating and contouring solely the neck in obese patients, therefore, can obtain satisfying aesthetic results
