Randomized prospective study comparing reverse and direct flow island flaps in digital pulp reconstruction of the fingers.

I read with interest the article by Dr. Braga-Silva and colleagues reporting the results of randomizing reverse versus direct island flaps for digital pulp re- construction in 122 patients to assess postoperative sensation and joint mobility. The direct flap resulted in better long-term discriminatory sensation, whereas the reverse flap resulted in better preservation of interphalangeal joint motion. Within the reverse flap group, nerve suture resulted in better sensation. The authors are to be congratulated on such a large series of patients that assesses two of the most important outcome parameters in digital pulp reconstruction.
