Discussion: The concept and method of closed reduction and internal fixation: A new approach for the treatment of simple zygoma fractures

The authors should be lauded for looking for less invasive methods of treating some zygoma fractures. There are many surgeons who, when confronted with a zygoma fracture, immediately use three or four surgical approaches to the fractured bone. The authors of this article, and others, have noted that the surgical approaches often used to treat such fractures can have more noticeable consequences than would a malre-duced zygoma. Ectropion, entropion, scleral show, sagging of the lateral lid margin, and others are all potential consequences of surgical approaches through or behind the lower eyelid. The incidence of such deformities varies but in one study was found to be 20 percent at 6 weeks after surgery. Therefore, the fewer surgical approaches needed, the less chance that soft-tissue deformities will occur.
