Abstract: Removal of lens epithelial cells (LECs) is known to reduce capsular phimosis and fibrotic capsular opacification. We describe a simple way of removing LECs during cataract surgery using the automated irrigation and aspiration system of any standard phacoemulsifi-cation machine. Occlusion is achieved by raising the vacuum to around 160 to 170 mm Hg while keeping the aspiration port of the irrigation and aspiration cannula against the posterior surface of the anterior capsule. Polishing is done by very small strokes of gentle circumferential movements on either side while maintaining the occlusion, and LECs can be seen to be cleaned off from the anterior capsule. Anterior capsular polishing by this method is not only quick, effective and safe, but also without the need of any extra instrumentation.
