Advanced Surgical Technique for Implantation of the Toric Implantable Collamer Lens

The incidence of cataract in Toric Implantable Collamer Lens (TICL) is reducing due to its newer design and better sizing using ultrasound biomicroscopy. Good surgical technique is essential to avoid cataract as well. To well prepare the patient, axis should be marked at the slit-lamp and the pupil should be dilated to 8 mm. Do not drape the operating eye until the lens is loaded. Do not touch the central part of the crystalline lens. Load and pull the lens with gentle continuous force. The corneal incision should be in the axis of the intended lens position to minimize lens manipulation. Do not overfill the anterior chamber with methylcellulose. Inject the lens into the anterior chamber and carefully dial the lens into the sulcus. Mark the correct axis on cornea and then adjust the lens position to align with the axis. The visual outcomes of TICL is very promising. Out of 44 cases of high myopic-astigmatic eyes 94% achieved UCVA of 20/25 or better.
