The aim of this study was to evaluate the relationship between relative component size and range of motion (ROM) after total knee arthroplasty (TKA). We retrospectively collected ROM and relative component size data from 401 primary TKA procedures. Three groups were created based on the relationship between femoral and tibial implant sizes, and then ROM at multiple time points was compared between groups. Subjects with larger femoral than tibial components had lower ROM than those with equally sized implants after 1 year (P<0.011) and after 3 years (P<0.023). Postoperative ROM was also compared between sexes within each of the 3 groups, but sex was not found to be a significant predictor of ROM. Therefore, we have observed that when a larger femoral than tibial component is used in TKA, long-term ROM may be limited in both males and females.
