What is the mechanism or mechanisms to explain the lower success of RTS in those who had undergone adjuvant RT? Does the lower success relate to the decreased mobility of the bulbomem-branous urethra after RT exposure or to the decreased external sphincter function after RT? Measureable magnetic resonance imaging variables by which the transobturator male sling might work were explored in a magnetic resonance imaging study. That study found that RTS failure was related to presence of periurethral fibrosis, whether occurring preoperatively or postoperatively. In another study by this same group,2 the clinical variables associated with RTS failure included weakened sphincter function, incomplete sphincter closure, and a lack of elongation of the coaptive sphincter zone.
