A 12-year-old boy with a right undescended testis presented for further evaluation after an incidental abnormal ultrasound finding. The undescended testis was first diagnosed at 1 year of age; however, he was lost to follow-up for over 10 years. At age 12, he presented to his pediatrician with a normal descended left testis and a palpably normal inguinal right testis. The pediatrician, concerned about the undescended testis, ordered a scrotal ultrasound that revealed an unremarkable inguinal right testis and a concerning 1.5 cm hypoechoic, hypervascular mass in the normally descended left testis (Fig. 1). At this point, the patient was referred to a pediatric urologist for further management.
