This paper addresses the important issue of whether, in the preoperative evaluation of submucous myomas, a 'Las-mar' score obtained by ultrasound alone can provide similar information to a score obtained using hysteroscopy. The authors undertook a well-conducted, prospective study with ultrasound evaluation and hysteroscopy being performed by different 'blinded' operators. They observed that three-dimensional (3D) transvaginal ultrasound alone provided results similar to those obtained by ultrasound combined with diagnostic hysteroscopy. The importance of this observation is that it is possible using ultrasound alone to anticipate surgical difficulty in certain cases and therefore either ensure appropriate surgical expertise, consider presurgical medical treatment of the myomas or use an alternative surgical procedure. This seems to fit the ultimate goal of achieving effective surgical outcomes whilst minimizing operative complications.
