Varenicline induced mania in a 51 year old patient without history of bipolar illness.

Varenicline is an alpha 4 beta 2 nicotinic receptor partial agonist with dopaminergic effects, approved for smoking cessation. Varenicline was reported to induce manic (Kohen and Kremen, 2007; Alhatem and Black, 2009), hypomanic (Morstad and Kutscher, 2008), or psychotic symptoms (DiPaula and Thomas, 2009) in 4 patients with bipolar illness, as well as a mixed mood and psychotic episode in a patient with depression (Pumariega et al., 2008). It was also reported to aggravate psychosis in a schizophrenic patient (Freedman, 2007), an adult with intellectual disability (Brahm, 2008) and to induce a mixed mood psychotic episode in a patient with schizoaffective disorder (Liu et al., 2009). We report a case of a varenicline-induced mania in a patient without a history of bipolar disorder.
