Sertraline-induced serotonin syndrome followed by mirtazapine reaction.

Serotonin syndrome is a potentially life threatening reaction to serotonergic drugs. We present the case of a man who developed serotonin syndrome on therapeutic sertraline, then suspected toxicity again on starting mirtazapine.The patient is a physically well 30 year old male who presented with his third episode of major depressive disorder in ten years, manifesting as low mood, anhedonia, poor sleep, poor appetite and feelings of guilt. The first episode resolved without medication, while the second episode was treated with sertraline. He had remained well without treatment for three years.
