Clozapine-induced sialorrhea (CIS) occurs in about 30% of patients treated with this medication (Praharaj et al., 2006). It is a socially stigmatizing adverse effect for the patients interfering with their quality of life and often leading to discontinuation of treatment. While non-pharmacological strategies to address CIS can be used alone in milder cases, in most cases it is difficult to treat and majority of the available pharmacological treatment options have either limited efficacy or unwanted side-effects (Gaftanyuk and Trestman, 2004). Glycopyrrolate is an anti-muscarinic drug, which is known for its potent drying property and is popular with the anesthesiologists as a preanesthetic agent (Blasco and Stansbury, 1996). It has been used to effectively treat chronic drooling in children and young adults with developmental delays (Blasco and Stansbury, 1996; Mier et al., 2000; Stern, 1997). As per a review on management strategies for clozapine-induced sialorrhea, there are no reports describing the use of glycopyrrolate for this adverse effect (Praharaj et al., 2006). A case of effective use of glycopyrrolate for clozapine-induced sialorrhea is described.
