It is well known that narcissistic personality disorder is one of the most difficult psychiatric disorders to treat. Much of the difficulty arises because of challenging patterns of transference and countertransference that develop in the course of treatment. Yet it is just these patterns of relatedness that must be the core of the treatment, since narcissistic patients have enormous problems in maintaining gratifying relationships.
