Clearly, the timely detection of fetal movement disorders (arthrogryposis has an incidence of 1 in 3-50008) should be improved. The current use of ultrasound techniques in early pregnancy focuses on evaluation of general growth and detection of aneuploidies, but it would be an excellent time to also search for fetal akinesia and contractures, particularly clubfeet and clenched hands. Visualization of any contractures should prompt complete evaluation of all joint positioning, the quality of fetal movements, lung development, muscle tissue mass and bone growth in the first and second trimester. Dedicating 45min to real-time ultrasound if clubfeet are found to observe joints and movement may be time well spent. The use of fetal MRI deserves special consideration. There are only sparse, but promising data.
