Reviewing fatal accidents that occurred at metal and non-metal mines in 2010, I was struck that blind and misplaced trust was repeatedly a prominent factor in incidents related to mobile equipment. In each case, a different mindset could have prevented what happened. All of us may sometimes ignore potential danger by telling ourselves that "nothing will happen - not now, not in the next few moments." And so workers will see equipment motionless and blindly trust that it will remain motionless. They will do this without having taken the required steps to ensure against motion.
