CROSS update: confidence is growing in reporting scheme

An international symposium in December 2006 was attended by representatives from several confidential reporting organisations - in health care, aviation and railways, and in the structural sector, CROSS (Confidential Reporting on Structural Safety). The most distinguished contributor was Linda Connell the legendary 'mother of the mother of all systems' (her own description) and director of the NASA ASRS (Aviation Safety Reporting System). This was established 30 years ago and has processed 750 000 reports since then. Her philosophy is: 1. Confidentiality is the first priority 2. Be a trusted honest broker who is external and independent 3. Have stakeholder participation to provide advice, guidance and constant advocacy 4. Ensure subject matter is given expert analysis/evaluation which increases credibility 5. Give feedback to the community on identification of high hazard events 6. Learn from the data.
