Legislation requires FDM, but even if it did not, open reporting in combination with FDM systems providing comprehensive safety information, is needed as a valuable'best practice' safety tool. A combined open reporting and FDM system will only functionrneffectively if it is seen to be part of a 'just culture'. Outcomes should, whenever possible, be positive in nature for the individuals involved, eg recommended re-training or support. It must not be seen as a remote disciplining tool.
